SaphenionPatientinfo: HIFU for varicose vein therapy – The SONOVEIN – System

SaphenionPatientinfo: HIFU for varicose vein therapy: High-intensity focused ultrasound (HIFU) is a medical application of ultrasound, in which targeted bundling of the sound waves heats and destroys tissue. This therapeutic method is also referred to as ultrasound ablation, focused ultrasound surgery or pyrotherapy. An important field of application is the treatment of prostate cancer, uterine fibroids, bone metastases.

Use in a variety of other diseases, e.g. in breast cancer, brain tumors, thyroid tumors is now also introduced.

SaphenionPatientinfo: HIFU for varicose vein therapy – Historical

The first investigations on the ablation of diseased tissue by HIFU were published in 1942. Further important work on this treatment method dates back to the 1950s and 1960s. Initially, HIFU was used predominantly in brain tumors; patients with Parkinson`s syndrome and other neurological disorders also found a non-invasive non-drug therapy solution in HIFU

HIFU use in neurosurgery was extensively explored in the 1950s and 1960s, but opportunities were limited by practical and technological limitations. From 1956 HIFU was widely used in the treatment of cancer.

From 1993, HIFU entered the non-surgical treatment of prostate cancer. From 2012 – 2015, HIFU was also approved for thyroid tumors and breast tumors.

SaphenionPatientinfo: HIFU for varicose vein therapy – use in cosmetics

At present, the HIFU procedure for use in skin tightening is being intensively discussed in the circles of the cosmetics industry and also among cosmetic surgeons. The „high-intensity-focused ultrasound“ treatment – HIFU for short – is now supposed to lift the face and flabby skin without scalpel and blood. Thread lifting or a conventional facelift with a scalpel should also be a thing of the past. There are already practices in Berlin as well as in Rostock – Warnemünde that use HIFU cosmetically.

With the face lifting or general tightening of the skin with HIFU, there are no cuts, scars or surgical pain. Thanks to the latest ultrasound technology, the face or area to be tightened should be smooth and firm again after just one treatment. The results are amazingly good, but the HIFU procedure is not completely painless as the skin and tissue warm up a bit.

SONOVEIN HIFU is a treatment that does not pose any high risks, especially for the elderly patient. Skin cuts are not necessary, no scars remain. The surrounding tissue (nerves, lymph vessels!) is spared. Medicines do not have to be discontinued.

Michel Huta, Vascular Surgeon and Venous Specialist explains the new SONOVEIN HIFU system for the treatment of varicose veins

The therapy is always outpatient, even in the treatment of truncal varicose veins a treatment of all veins in only one session is possible. This is similar to the therapy with the vein glue VenaSeal.

The duration of treatment varies between 20 and 120 minutes, depending on the level of chronic venous disease and the truncal or perforator vein findings. We do not need a sterile surgical tract, there is no risk of post-operative infections.

The side effects are significantly reduced compared to the other treatment procedures, especially radical stripping surgery. A slight tissue swelling and a short-term feeling of warmth are described. This is roughly comparable to the condition after the radio wave, but without a catheter, without skin incision, without a scar.

After the treatment an immediate return to everyday life is possible – as well as after the therapy with the vein glue.

SaphenionScience: SONOVEIN – the European approval

SONOVEIN® is a Class IIb medical device developed and marketed by Theraclion for the transcutaneous (skin surface) coagulation of incompetent veins in patients with superficial venous reflux (truncal varicose veins, saphenous veins, perforantor varicosis). This medical device is a regulated health product that is CE Marked 2797 under this regulation.

Since April 2019, the medical device SONOVEIN® has had the o.g. CE certificate for the treatment of varicose insufficient veins.

SaphenionPatientinfo: SONOVEIN – HIFU is the latest addition to minimally invasive treatment options. Manufacturer is the French company Theraklion. SONOVEIN is based on a unique ultrasound therapy technology that combines focused ultrasound therapy (HIFU) and real-time ultrasound monitoring.

SONOVEIN® is a computer-controlled medical device for the treatment of tissues that are not accessible from the outside, with high-intensity focused ultrasound (HIFU). In the treatment head of the device is a probe that gives off energy, and an imaging system that makes the tissue visible before and especially during treatment.
The side effects are significantly reduced compared to the other treatment procedures, especially radical stripping surgery. A slight tissue swelling and a short-term feeling of warmth are described. This is roughly comparable to the condition after the radio wave, but without a catheter, without skin incision, without a scar.

This dual function enables a precise, individualized, non-invasive and outpatient treatment of varicose veins. The high-energy ultrasound waves penetrate the skin and are focused on an area in the target tissue. The tissue in this area becomes hot and dies.

The high-intensity ultrasound beam is focused on the vein, much like when light is focused through a magnifying glass.
When the thermal energy is so focused, the vein shrinks and closes, similar to endothermic but non-invasive methods.
Treatment along the vein is automatically controlled via a touch screen.
A second ultrasound probe allows visualization of the vein in real-time and ensures optimal accuracy.

As described many times on the website of Saphenion, we also feel, although coming from the surgical / vascular surgical field, very close to the minimally invasive therapy of vascular diseases. On the one hand, this concerns the catheter-supported interventions on the arterial system – here we have been working with the catheter since 1995.

But it also affects the venous system, especially the varicose veins. These have been treated by Saphenion since 2002 with the catheter. First, we started with the laser, then switched to the radio wave catheter in 2007 and started in 2012 with the vein glue VenaSeal. Here we have treated 2454 veins with the vein glue in the last more than 7 years.

It is up to us to achieve the same high varicose vein occlusion effectiveness with the SONOVEIN – HIFU as we have been using for 87 months with the vein glue – a closure efficiency of 96.2% over more than 7 years of use!
