Der Beginn einer Revolution
Im Jahr 2013 begannen Dr. Enric Roche und Dr. Antoni Puig in der Clínica Vascular Barcelona mit einem Prototyp zu arbeiten, der aus einem sterilen Probenbehälter bestand, der mit einem speziell angefertigten Labormagneten (JP Selecta) ausgestattet war. Wenn das Sklerosierungsmittel hinzugefügt und das Rührsystem auf hohe Geschwindigkeit eingestellt wurde, entstand ein hochwertiger Schaum.
Auf dem European Venous Forum 2014 in Paris stellte Dr. Roche die Ergebnisse einer Vergleichsstudie vor, in der der mit dieser neuen Methode erzeugte Schaum dem nach der traditionellen Methode (Tessari-Methode) erzeugten Schaum gegenübergestellt wurde.
Die Schlussfolgerungen lauteten, dass dieses neue System bei der Herstellung von Schaum aus Luft oder physiologischen Gasen wirksam war. Im Vergleich zur manuellen Methode (Tessari) war der erzeugte Schaum homogener, stabiler und konnte zudem bereits aus sehr geringen Konzentrationen des Sklerosierungsmittels hergestellt werden.
The start of a revolution
In 2013, in Clínica Vascular Barcelona, Dr. Enric Roche and Dr. Antoni Puig began working with a prototype consisting of a sterile sample pot with a customised laboratory magnet inside it (JP Selecta). When the sclerosing agent was added and the stirring system set at a high number of revolutions, it produced a high-quality foam.
At the 2014 European Venous Forum in Paris, Dr Roche presented the results of a comparative study of the foam created with this new method against that created with the traditional method (Tessari method). The conclusions were that this new system was effectve preparing foam made from air or physiological gases. In comparison with the manual method (Tessari) they were more homogeneous, more stable, and moreover could be prepared even from very low concentrations of sclerosing agent.

Saphenion®: 139 Monate Mikroschaumverödung an Stammkrampfadern: Unsere Ergebnisse
Seit März 2002 haben wir 23828 duplexgesteuerte Mikroschaumtherapien durchgeführt. In den letzten 139 Monaten (1 / 2015 – 06 / 2026) wurden bei Saphenion® in 11769 Fällen Klebender Mikroschaum (Sealing Foam) für die Therapie der sichtbaren Krampfadern unterschiedlicher Lokalisation gewählt.
Bei 2996 Patienten wurde eine Stammvaricosis der Beine behandelt. Im Einzelnen betraf dies in 1037 Fällen die V. saphena parva, in 564 Fällen die V. saphena magna und in 1399 Fällen akzessorische Stammvenen an Oberschenkel und Unterschenkel. Das Durchschnittsalter der Patienten betrug 59 Jahre (15 – 94 Jahre).
Die Therapie einer inkompletten Stammvaricosis ist im Zeitraum von 139 Monaten in 95,5% erfolgreich. Die Vena saphena parva zeigt sich in 93,06 % verschlossen. Die Vena saphena magna war, über den gleichen Zeitraum betrachtet, in 89,05% verschlossen. Die akzessorischen Stammvenen waren – nach Unterbrechnung des Zuflusses aus der Leistenmündung oder dem vorherigen Verkleben der defekten Stammkrampfader – in 95 % verschlossen).
Dies entspricht einer Gesamtverschlussrate von 93,1% für alle therapierten Venen.
Saphenion®: 139 Months of Microfoam Sclerotherapy for Truncal Varicose Veins: Our Results
Since March 2002, we have performed 23,828 duplex-guided microfoam sclerotherapy procedures. Over the past 139 months (January 2015 – June 2026), Saphenion® selected Sealing Foam for the treatment of visible varicose veins in various locations in 11,769 cases.
Truncal varicosis of the legs was treated in 2,996 patients. Specifically, this involved the v. saphena parva in 1,037 cases, the v. saphena magna in 564 cases, and accessory trunk veins in the thigh and lower leg in 1,399 cases. The average age of the patients was 59 years (range: 15–94 years).
Treatment of incomplete truncal varicosities was successful in 95.5% of cases over a period of 139 months. The small saphenous vein was occluded in 93.06% of cases. The great saphenous vein was occluded in 89.05% of cases over the same period. The accessory trunk veins were occluded in 95% of cases after interrupting the inflow from the inguinal junction or following prior sclerotherapy of the defective trunk varicose vein.
This corresponds to an overall occlusion rate of 93.1% for all treated veins.
Saphenion®: 139 Monate Mikroschaumverödung an Stammkrampfadern: Unsere Erfahrungen bei 2996 Patienten
Mit der Modifikation der Mikroschaumverödung zum Klebenden Mikroschaum (Sealing Foam) sehen wir in den letzten Jahren eine wesentlich verbesserte Verschlusseffektivität. Auch die häßlichen braunen Striche und Flecken auf der Haut (Ecchymosis) nach der Therapie sind seltener. Hämatome unter der Haut an den Punktionsstellen sind in < 8% aufgetreten. Venenentzündungen haben wir nach der Behandlung an 23825 Patienten in 119 Fällen (0,5% – mehrere Fälle nach Covid – Impfung!) gesehen und entsprechend nachbehandelt.
Eine Venenentzündung nach der Mikroschaumtherapie tritt nach unseren Erfahrungen auch deutlich seltener auf, wenn der Patient begleitend zur Mikroschaumverödung nach der Injektion des Mikroschaums in die Venen eine Vacuum – Kompressionsmassage der Beine und eine begleitende Sauerstoff – Mehrschritt – Therapie (SMT) erhält. Wir haben nach SMT – Therapie nur noch in 39 Behandlungsfällen (0,16%) eine oberflächliche Hautvenenthrombose (OVT) gesehen.
In lediglich 16 Fällen (0,06%) mußten wir offene Stellen (Punktionsulcus) nachbehandeln. In keinem Fall wurde von unseren Patienten Sensibilitätsstörungen oder Taubheitsgefühl nach der Behandlung beschrieben.
Im Wesentlichen berichten unsere Patienten neben den sichtbaren kleinen Punktionshämatomen in erster Linie von leichtem Stranggefühl, Gnubbelbildung und Druckschmerz über der behandelten Vene. Auch muskelkaterähnliche Erscheinungen traten auf (dies ähnelt den Symptomen nach Therapie mit dem VenaSeal® – Venenkleber).
Bei 20 Patientinnen und 5 Patienten erlebten wir in der Praxis leichte Migräneattacken (0,1%) nach der Behandlung. Nach 30 Minuten Ruhezeit in unserem Aufwachraum mit einem Glas Wasser mit Traubenzucker und Einnahme eines Migränehemmers konnten alle Patienten die Praxis selbstständig verlassen.
Auch aus diesem Grund haben wir aktuell seit dem 11. 06. 25 das Varixio – System in der Praxis getestet und nunmehr eingeführt. Insbesondere der stickoxidarme Mikroschaum ergibt für unsere Migränepatienten völlig neue Therapiemöglichkeiten! Bisher sind 203 Patienten mit diesem neuen System therapiert worden (6,77% aller Patienten nach Mikroschaumtherapie an Stammkrampfadern).
Weiterhin haben wir in den letzten 12 Monaten auch keine Hautvenenthrombosen mehr gesehen und auch keine Punktionsulcera.
Und das Ergebnis hat uns noch einmal sehr positiv überrascht:
Saphenion®: 139 Months of Microfoam Sclerotherapy for Truncal Varicose Veins: Our Experience with 2,996 Patients
With the modification of microfoam sclerotherapy to sealing foam, we have observed a significantly improved occlusion rate in recent years. The unsightly brown streaks and spots on the skin (ecchymosis) following treatment are also less common. Subcutaneous hematomas at the puncture sites occurred in < 8% of cases. We observed 119 cases of phlebitis (0.5%) following treatment in 23,825 patients—several of which occurred after COVID-19 vaccination—and provided appropriate follow-up treatment.
In our experience, venous inflammation following microschaum therapy also occurs significantly less frequently when the patient receives vacuum compression massage of the legs and concomitant multi-step oxygen therapy (SMT) in addition to the microschaum sclerotherapy after the microschaum is injected into the veins. Following SMT therapy, we have observed superficial skin vein thrombosis (OVT) in only 39 treatment cases (0.16%).
In just 16 cases (0.06%), we had to treat open wounds (puncture ulcers). None of our patients reported sensory disturbances or numbness following treatment.
Essentially, in addition to the visible small puncture hematomas, our patients primarily report a slight sensation of tightness, lump formation, and tenderness over the treated vein. They also experience muscle soreness-like also occurred (this is similar in the symptoms following with VenaSeal® Vein glue.
Twenty female patients and five male patients experienced mild migraine attacks in our practice following treatment (0.1%). After a 30-minute rest period in our recovery room, during which they were given a glass of water with glucose and took a migraine medication, all patients were able to leave the practice on their own.
For this reason as well, we have been testing the Varixio system in our practice since June 11, 2025, and have now implemented it. In particular, the low-nitric oxide microfoam opens up entirely new treatment options for our migraine patients! To date, 203 patients have been treated with this new system (6.77% of all patients who underwent microsponge therapy for truncal varicose veins).
Furthermore, over the past 12 months, we have not seen any cases of cutaneous vein thrombosis or puncture ulcers.
And the results have once again pleasantly surprised us!
